Three generations of the Maddox family, right here in Alabama. We've been caring for Gadsden and Boaz families since 1982 — and we treat every patient like one of our own.
No insurance? We still have affordable options for you.
Whether you're in Gadsden or Boaz, your family has a dental home nearby with the same doctors and the same caring team.
Dr. Maddox founded this practice in 1982 and raised his family here — including two daughters who are now dentists themselves. When you walk in, you're joining a tradition built on trust, relationship, and genuine care.
From your child's first visit to grandma's dentures — we're your one-stop dental home.
No surprises, no pressure. Just a warm welcome and a thorough, unhurried first visit.
"Dr. Maddox has been our family's dentist for over 20 years. Now Dr. Shadix sees our kids — it really does feel like a family taking care of a family. I wouldn't go anywhere else."
"I hadn't been to the dentist in years and was honestly embarrassed. Dr. Spruiell in Boaz made me feel completely at ease — zero judgment, just kind. Fixed up my teeth over a few visits. I wish I'd come sooner."
"The staff here has been the same faces for as long as I can remember. That kind of consistency says everything. They know my whole family, my history, and they genuinely care. Truly the best dental practice around."
Not sure if we take yours? Just call — we're happy to check for you before your visit.
No insurance? We believe everyone deserves quality dental care. Ask us about our affordable payment options when you call.
Call whichever location is most convenient. Our team will get you on the schedule quickly, answer insurance questions, and make sure you feel prepared for your visit.
Whether you're calling about a toothache at 8am or planning a complete smile makeover, we have the expertise — and the heart — to take care of you.
Regular cleanings, comprehensive exams, digital X-rays, and early detection of issues before they become costly. We personalize your cleaning schedule to your specific needs — not a one-size-fits-all protocol.
Tooth-colored composite fillings blend seamlessly with your natural teeth. We remove only what's necessary and restore the tooth to full function — conservatively and carefully.
When a tooth is cracked, broken, or significantly decayed, a crown restores it to full strength and appearance. Bridges replace missing teeth without surgery. We use quality materials matched to your natural smile.
Modern root canal treatment is far more comfortable than its reputation. We numb the area thoroughly, remove the infected tissue, and seal the tooth — so you keep your natural tooth and leave pain-free.
We believe esthetics and health go together. When your smile looks great, you feel great — and we make it happen without the premium-clinic price tag.
Porcelain veneers cover chips, stains, gaps, and uneven shapes to create a stunning, natural-looking smile. Our doctors design each veneer to complement your unique facial features and bite.
Professional whitening delivers dramatically brighter results than over-the-counter products. We offer in-office and take-home options depending on your timeline and desired outcome.
Implants look, feel, and function like your natural teeth. A titanium post integrates with your jawbone and supports a beautiful crown — permanently. No slipping, no adhesives, no bone loss over time.
Full and partial dentures crafted to fit your mouth comfortably and look completely natural. Dr. Allen's expertise in prosthodontics means your dentures are designed with precision and long-term wearability in mind.
Call us first thing in the morning and we'll do everything we can to get you seen the same day. Don't tough it out — dental emergencies rarely improve on their own.
We treat: severe toothaches, chipped or broken teeth, knocked-out teeth, lost crowns or fillings, dental abscesses, and swollen jaw or face.
We accept most major plans including Delta Dental, Cigna, Aetna, MetLife, United Healthcare, and BCBS Alabama. Not sure? Just call — we'll check your coverage before your visit so there are no billing surprises.
You're not alone — and you won't be judged here. We see patients in all stages of dental health and our only goal is helping you get back on track comfortably. We start wherever you are and move at your pace.
Absolutely — we love seeing kids. We recommend bringing children in by age one or when their first teeth appear. Our team is experienced with nervous little patients and knows how to make dental visits fun rather than scary.
A warm welcome, comprehensive exam, X-rays, and a thorough review with your doctor. We'll walk you through everything we find — clearly, without jargon — and present any treatment recommendations without pressure. Plan for about 90 minutes for your first visit.
Dental anxiety is very common and completely understandable. Tell us when you call — our team is experienced at helping nervous patients feel calm and in control. We move at your pace, explain every step before we do it, and stop anytime you need a moment.
Whichever is most convenient for you. Both offices have the same doctors and the same team. Some of our doctors split time between locations — if you want to see a specific doctor, just ask when you call and we'll match you up at either location.
Five doctors — four of them family. A staff that has been with us for over 27 years. We didn't set out to build a dental practice. We set out to serve our community, and the practice grew from that.
513 South 3rd Street
Home since 1982
Dr. Mike Maddox graduated from UAB School of Dentistry in 1982 and came straight home to Gadsden to open his practice on South 3rd Street. In the early days, he and his wife Cindy lived on one side of a duplex while the practice ran on the other. The practice — and the family — quickly outgrew that arrangement.
Today, two of Dr. Maddox's daughters — Dr. Mary Lynn Shadix and Dr. Caroline Spruiell — practice alongside him. Dr. Caroline's husband Robby, who grew up in Boaz and returned to serve his own community, rounds out the family team. And Dr. Roger Allen, a Gadsden native with a remarkably varied background, brings deep expertise in prosthodontics.
"We consider it a great privilege to be your dental home and to take care of you and your family."
— Family Dentistry AssociatesMany of our staff members have been with us for over 27 years. When you walk in, you'll recognize familiar faces — and they'll recognize yours.
Each brings their own expertise and story — and all share the same commitment to this community.
Born and raised in Gadsden, Dr. Maddox graduated from UAB School of Dentistry in 1982 and immediately came home to open his practice on South 3rd Street. In the early days, he and his wife Cindy literally lived on one side of the office duplex. The practice grew, the family grew, and both have called South 3rd Street home ever since. He is the father of Dr. Mary Lynn Shadix and Dr. Caroline Spruiell — a legacy he carries with visible pride.
Dr. Shadix followed her father's footsteps — the University of Alabama for biology, then UAB School of Dentistry, graduating with her DMD in 2017. She and her husband David returned to Gadsden shortly after to start their family and continue building the practice her father started. Serving the community she grew up in, working alongside her dad and sister, is something she describes as a genuine privilege.
Dr. Caroline Spruiell earned her BS in Biology at Alabama in 2014, followed by a Master of Science in Biomedical and Health Sciences from UAB, and then her DMD from UAB School of Dentistry in 2020. She practices at both the Gadsden and Boaz offices alongside her father, sister, and husband — a uniquely wonderful family arrangement that brings a whole new generation to the practice Dr. Maddox built.
A Boaz native, Dr. Robby Spruiell earned his BS and MS in Microbiology from Auburn University before spending a year doing mission work in East Asia. He then completed his DMD at UAB in 2020 and returned home to practice with his father-in-law, wife, and sister-in-law. For him, dentistry in Boaz isn't just a career — it's giving back to the community that raised him.
Dr. Allen's path to dentistry is one of the most remarkable in the field. A Gadsden native, he worked at Burger King in East Gadsden before attending UAB for his BS, then spent years as a Chemical Research Technician at Southern Research Institute — researching everything from pizza to chemical warfare, carpet to cancer. He went on to investigate periodontal bacteria at UAB for six years, then earned his DMD from Pittsburgh School of Dental Medicine in 1996. After completing his residency in Prosthodontics at UAB (where he served as Chief Resident and floor instructor), he returned to Gadsden in 1999 and has been serving the community for over 26 years. Outside dentistry, Dr. Allen channels his creativity into cooking, photography, costume and prop making, woodworking, leatherworking, and metalworking.
30 years and two generations of the Maddox family. Same street. Same commitment. Two locations, one family, and a team that truly cares about you.
